Make sure everything was replaced as a kit (pressure plate, clutch plate and thrust bearing) and that the flywheel was skimmed before re-installation. A common place for them to put it is under the carpet under one of the front seats.
As the battery is rather heavy they generally don't put them in the dash etc. Have a feel around the floor, you might get lucky.

DOT4 is what Toyota reccomend for our vehicles, so that is what I use.

While we are talking now about over flowing resevoirs, my 2cW. Your brake system has been designed to hold a certain amount of brake fluid when the pads and shoes are new. As these wear down the brake fluid drops in the resevoir. Now most people will fill up the resevoir when they see it is dropping...

Have your ABS sensors been cleaned? It could be sending false reading to the computer and causing the lock-up.
They are normally covered in break dust, so I guess mud would make a difference.

I am fairly sure it is about the same amount of work as with our SFA. You have to strip the wheel right down to taking the disc off. That means stripping the free wheel hubs and loosening the bearings. Once the bearings are loose, the disc and hub can be removed. The studs are fitted from the inside...

The right hand side could be leaking slightly, hence the pulling to the left (the side where there is more pressure)
