4.0 V6 Paranoia

I recently noticed two things on my 2008 4.0 V6 Fortuner:
The first being a vibration that may have always been there and that I just never noticed. When I hold the revs at 2500-3000, in neutral, that there is a vibration throughout the vehicle and you can feel it on the door and roof too from the outside. Almost as if it is missing. This goes away when you rev it higher in neutral or you drive it.
The other thing that has happened of late is that when the engine is warmed up properly, there is a clack-clack-clack sound. Something like a tappet/lifter/leaking exhaust gasket type of sound. This noise is absent when cold or warm and gets noisy when warmed up. My wife also asked me why it is making a noise the other day, so this is obviously new.
Is this normal or does anyone else have the same issue? I took it to a very well respected mechanic and he reckons just about all the V6's he works on sound and feel the same. He reckons because of the design of the valves seating and the factory rather giving more clearance, it should technically get quieter and not noisier. The bus goes like the stink and there is absolutely no difference in power or fuel consumption. The vehicle has not missed a service and has 110 000km on the clock.
